- Perform evaluations and develop individualized treatment plans to improve mobility, strength, and function.
- Provide direct therapy services, including exercises, manual therapy, and therapeutic modalities.
- Collaborate with an interdisciplinary team to create comprehensive care plans that align with residents’ goals and needs.
- Educate residents and their families on treatment plans and home exercise programs to support recovery and long-term success.
- Maintain accurate and up-to-date documentation in compliance with facility guidelines and state regulations.
- Doctorate in Physical Therapy (DPT) or equivalent from an accredited program.
- Active Pennsylvania Physical Therapy license (or eligibility to obtain).
- Experience in a skilled nursing facility or long-term care setting preferred but not required.
