Responsibilities:
- Perform thorough evaluations of residents to assess their physical conditions and mobility challenges.
- Develop personalized treatment plans to restore function, reduce pain, and prevent disability.
- Utilize a variety of therapeutic exercises, manual therapy, and modalities to meet each patient’s goals.
- Collaborate with nurses, physicians, and other healthcare staff to ensure comprehensive patient care.
- Educate patients and families on post-treatment exercises and strategies to maintain progress.
- Accurately document patient progress and maintain records in compliance with regulatory standards.
Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent.
- Active licensure as a Physical Therapist.
- Experience in a skilled nursing or long-term care setting preferred.
- Strong interpersonal and communication skills to work effectively with patients, families, and healthcare teams.
