This position is ideal for someone with at least one year of SNF (skilled nursing facility) experience who is passionate about working in neuro rehab and is ready to make a difference in a long-term care setting.

Key Responsibilities:
- Develop and implement personalized treatment plans for residents with neurological conditions.
- Conduct assessments to identify residents' needs and create goals to enhance their functional independence.
- Collaborate with a multidisciplinary team to provide comprehensive care.
- Educate residents and families on therapy strategies for long-term progress.
- Maintain accurate, detailed documentation for all treatments and interventions.
Requirements:
- Licensure: Current Colorado OT license in good standing.
- Experience: Minimum of 1 year of experience in a skilled nursing facility (SNF) setting.
- Strong communication and interpersonal skills.
