Temp Medicare Certified Home Health Registered Nurse (RN) OPPORTUNITY * Boca Raton, Florida * $47-$50/hr

Job Summary: We are seeking a skilled and compassionate Medicare Certified Home Health Registered Nurse (RN) to join our team. The ideal candidate will have a strong background in home health care, a thorough understanding of Medicare regulations, and a passion for providing exceptional patient care. This role involves assessing patient needs, developing and implementing care plans, and providing ongoing evaluations and education.

Key Responsibilities:
  • Schedule: Monday-Friday 8a-4:30pm. Must be available to take call every other weekend. Call is Monday-Friday 4:30p-8a and 24 hour call for Saturday and Sunday. 40 hours guaranteed.
  • Patients per day: Will have multiple patients scheduled each day to be seen at 2 different communities within 4 miles of each other. Must have reliable transportation.
  • Patient Assessment: Conduct comprehensive assessments of patients' health status, including physical, emotional, and social aspects, to develop individualized care plans.
  • Care Planning: Create, implement, and revise care plans in collaboration with patients, families, and interdisciplinary team members, ensuring alignment with Medicare guidelines.
  • Medication Management: Administer medications, monitor patient responses, and educate patients and families on medication management and potential side effects.
  • Wound Care: Provide skilled nursing care for wound management, including dressing changes and monitoring for signs of infection.
  • Patient Education: Educate patients and families on disease processes, home care techniques, and preventive health practices to enhance self-management and improve outcomes.
  • Coordination of Care: Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and effective patient care.
  • Documentation: Maintain accurate and timely documentation of patient care, progress, and changes in condition, in accordance with Medicare and agency policies.
  • Compliance: Ensure adherence to Medicare regulations and agency policies, including quality assurance and performance improvement initiatives.
  • Emergency Response: Respond to patient emergencies promptly and effectively, utilizing critical thinking and clinical judgment.

Qualifications:
  • Education: Associate's or Bachelor’s degree in Nursing from an accredited institution.
  • Licensure: Valid Registered Nurse (RN) license in the state of Florida.
  • Certification: Current BLS (Basic Life Support) certification; additional certifications (e.g., ACLS) may be preferred.
  • Experience: Minimum of 1-2 years of recent nursing experience in home health care, with a strong understanding of Medicare guidelines and home health care regulations. Must be experienced in completing Oasis evaluations.
  • Skills: Excellent clinical skills, strong communication abilities, and proficiency in electronic medical records (EMR) systems.
  • Attributes: Compassionate, detail-oriented, with strong problem-solving skills and the ability to work independently while managing a diverse caseload.
  • Transportation: Must have reliable transportation.
