- Conduct intake assessments to identify each client’s needs, history, and treatment goals.
- Develop and implement personalized treatment plans in collaboration with the clinical team.
- Facilitate individual and group therapy sessions, offering guidance and support to clients.
- Provide crisis intervention and case management, ensuring clients have access to necessary resources.
- Advocate for clients and connect them to community resources for ongoing support post-discharge.
- Document progress and collaborate closely with the healthcare team for seamless care.
- Master’s degree in Social Work (MSW) and a valid state license (LCSW or equivalent)
